Insurance   

 

We currently accept the following insurance plans:​

• CIGNA

• HORIZON BLUE CROSS BLUE SHIELD

 

Nutrition counselling is covered under many insurance plans!

Please contact your provider PRIOR to your consultation to find out your plan's coverage.

​

Some helpful questions include:

• Does my plan cover nutrition counseling (specifically CPT codes 97802 and 97803)?

• How many sessions are covered per calendar year?

• Do I have a co-pay?

• Do I need a referral?

Co-pays are due at time of  consultation. We will bill your insurance provider and you will be responsible for the balance of the payment.

Payments

​

If your insurance plan does not include nutrition counseling full payment will be collected at the time of your consultation. For your convenience, we accept cash, check or credit card as payment. At the end of your appointment, we will provide you with a Super Bill to submit to your insurance company for possible reimbursement.
